Required Documents

 

The following documents are required by the insurance payer:

 

  • Insurance claim form

  • Copy of the policy

  • Bills for exams and procedures

  • Receipts for prescription drugs

  • Medical records and progress notes

  • Copy of passport and identification card

 

 

How to File Claims

 

  • Reimbursement for out-of-pocket payments:
    Submit all invoices and receipts related to the medical care to TASCO along with the required documents listed above.

  • Claims for visits to emergency rooms:
    In most cases, hospitals bill insurance companies directly after emergency room visits. If you have visited an emergency room, submit to TASCO a copy of the insurance policy, insurance claim form, and a privacy policy consent form as soon as possible in order to expedite the claims process.

 

 

Other Important Information

 

  • If you have misplaced the insurance policy document, contact your insurance sales agent or the LIG customer service.

  • We only process healthcare-related claims. Loss/theft claims must be sent to the LIG corporate office in Korea.

  • If the patient is a minor, the claim form must specify the legal guardian. Also specify the beneficiary for reimbursement.

  • For the claims from outside of the United States, provide us with the bank name, bank address, SWIFT code, account number, and account holder name,
